Medication Authorization form

To all Parents

As the school year begins, I would like to remind parents of some important issues that need to be addressed that will help us and help keep your children healthy in school.

Health history and medications orders MUST be completed or updated every school year. Please let the school nurse know if your child has significant health history, life threatening allergies or special medical concerns. Contact the school nurse any time during the year when there are changes or newly developed health conditions. Communication is the key to making sure children’ health needs are met during the school day.

When a child requires medication, in most cases this can be given at home, outside the regular school hours.  We encourage parents and physicians to minimize the prescribing of medication to be taken during the school day. However, if your child must receive medication in order to be in school, please completed the Medication Authorization Form and return back to nursing station.

School personnel will NOT administer any medications to students unless they have received the Medication Authorization Form properly completed and signed by the parent/guardian. All medications must be labeled & received in the original container.

Any medication should be brought to school by the parent or parent’s representative. It should be delivered personally to the school nurse/school health professional, class teacher or welfare (or medical) assistant.  All medications must be clearly labelled with contents, owner’s name, dosage and route. Medication will not be sent home with a child. Where there is an excess of medication sent, these MUST be collected from the school by an adult. The first dose of any new medication CANNOT be given at school.  Any potential side effects must be monitored at home.
